老年食管癌放疗患者感染病原菌分布及对环氧酶2和上皮钙粘附蛋白的影响调控研究

  • 摘要: 目的 研究老年食管癌放疗患者感染病原菌分布及对患者血清中环氧酶2(COX2)和上皮钙粘附(Ecadherin)蛋白影响,为临床诊断、治疗和患者预后判断提供参考。方法 对医院2012年2月-2015年8月诊治200例老年食管癌放疗感染患者进行分析并将其设为感染组,分析患者感染病原菌的分布,酶联免疫吸附测定法检测感染后患者血清COX2、Ecadherin和B淋巴细胞瘤2基因/B淋巴细胞瘤2基因相关蛋白/(Bax/Bcl2)以及X蛋白结合蛋白(XBIP)和基质金属蛋白酶水平的变化,以医院同期健康体检者200例为对照组,老年食管癌放疗未感染患者200例设为未感染组。结果 200例感染患者中共分离病原菌200株,其中革兰阳性菌102株,占51.0%,革兰阴性菌98株,占49.0%;感染患者血清Bcl2、MMP2、COX2、Ecadherin和XBIP蛋白水平明显高于对照组和未感染组患者而Bax和C-myc蛋白明显低于对照组,且差异有统计学意义(P<0.05)。结论 老年食管癌放疗患者容易并发病原菌感染,且此过程与患者血清中环氧酶2和上皮钙黏蛋白水平异常相关。

     

    Abstract: OBJECTIVE To study pathogen distribution and its effect on serum COX2 and Ecadherin in esophageal carcinoma patients who received radiotherapy so as to provide references for clinical diagnosis, treatment and prognosis. METHODS Totally 200 cases of esophageal carcinoma patients who received radiotherapy in our hospital form Feb. 2012 to Aug. 2015 were enrolled in this study. The genotypes and distribution of pathogens were identified by VITEK-2 Compact equipment. The serum COX2 and Ecadherin as well as MMP2, C-myc and Bax/Bcl2 were detected by ELISA. A total of 200 cases of healthy subjects and 200 cases of uninfected patients at the same period in our hospital were taken as control. RESULTS The pathogens isolated from the patients included 102 (51.0%) gram positive bacteria and 98 (49.0%) gram negative bacteria. The serum Bcl2, MMP2, Ecadherin and XBIP increased greatly in infected patients while the Bax and C-myc proteins decreased significantly when compared with control. The differences were significant (P<0.05). CONCLUSION Esophageal carcinoma patients received radiotherapy suffer from pathogen infection and the abnormal expression of COX2 and Ecadherin are involved in this pathological process.
