术前口腔准备预防全麻后气管插管患者下呼吸道感染的效果分析
Effect of preoperative oral care on prevention of lower respiratory tractinfections in endotracheal intubation patients after general anesthesia

摘要: 目的 分析不同口腔准备措施对气管插管全身麻醉后下呼吸道感染发生率的影响,为临床预防提供参考依据。方法 选取2013年6月-2014年9月全身麻醉下行腰椎后路椎管减压椎间融合术的气管插管患者330例为研究对象,随机分为3组,每组各110例,术前分别采取刷牙、含漱液漱口、联合措施等不同的口腔准备措施,同时选取历史空白对照100例,分析术前不同的口腔准备对全麻后下呼吸道感染的预防效果。结果 刷牙组下呼吸道感染率16.51%,含漱液组下呼吸道感染率10.09%,联合措施组下呼吸道感染率4.59%,空白对照组下呼吸道感染率20.00%,4组间下呼吸道感染发生率差异有统计学意义(P<0.05),其中联合措施组全麻后下呼吸道感染率与刷牙组、空白对照组比较差异均有统计学意义(P<0.05);其余组间比较,下呼吸道感染率差异无统计学意义。结论 全麻前常规刷牙加复方氯己定含漱液漱口,对气管插管全身麻醉后下呼吸道感染的预防有一定的效果。Abstract: OBJECTIVE To explore the influence of the oral care measures on incidence of lower respiratory tract infections in endotracheal intubation patients after general anesthesia so as to provide guidance for clinical prevention.METHODS A total of 330 endotracheal intubation patients who underwent posterior lumbar spinal decompression interbody fusion surgery under general anesthesia from Jun 2013 to Sep 2014 were recruited as the study objects and randomly divided into three groups,with 110 cases in each group.The preoperative oral care measures,including the tooth brushing,gargle,and combined measures,were respectively taken,100 cases were set as the blank control group,and the effects of the preoperative oral care measures on prevention of the lower respiratory tract infection were observed.RESULTS The incidence of lower respiratory tract infection was 16.51%in the tooth brushing group,10.09%in the gargle group,4.59%in the combined measure group,20.00%in the blank control group,and there was significant difference in the incidence of lower respiratory tract infection among the four groups(P<0.05);there was significant difference in the incidence of lower respiratory tract infection among the combined measure group,the tooth brushing group,and the blank control group after the general anesthesia(P<0.05);however,as compared with other groups,there was no significant difference in the incidence of respiratory tract infection.CONCLUSION The application of conventional tooth brushing plus mouth washing with chlorhexidine gargle before the general anesthesia can achieve certain effect on prevention of the lower respiratory tract infection in the endotracheal intubation patients after the general anesthesia.
