层粘连蛋白和乙型肝炎表面抗原在乙型肝炎相关性肝癌组织中的表达及相关性研究
Expression and correlation studies of LN and HBsAgin the tissues of HBV-related hepatocellular carcinoma

摘要: 目的 探讨层粘连蛋白(laminin,LN)和乙型肝炎表面抗原(HBsAg)在乙型肝炎相关性肝癌组织中的表达及相关性,为乙型肝炎相关性肝癌的临床诊治提供参考依据。方法 选取2014年1月-2015年12月在医院行手术治疗50例乙型肝炎相关性肝癌及50例良性肝脏疾病患者,采用免疫组化法对肝癌及癌旁正常组织、良性肝脏疾病组织中LN及HBsAg表达进行检测,采用化学发光法测定血清LN。结果 肝癌组织、癌旁正常组织、良性肝脏疾病组织细胞膜/细胞质中LN表达阳性率分别为28.00%、8.00%、4.00%,组织肝窦及新生血管中LN表达阳性率分别为66.00%、54.00%、8.00%,肝癌组织细胞膜/细胞质中、肝窦及新生血管中LN表达阳性率明显高于癌旁正常组织及良性肝脏疾病组织(P<0.05),肝癌组织、癌旁正常组织肝窦及新生血管中LN表达阳性率高于细胞膜/细胞质中(P<0.05),肝癌患者血清LN水平高于良性肝脏疾病患者(P<0.05);肝癌组织、癌旁正常组织、良性肝脏疾病组织HBsAg表达阳性率分别为42.00%、70.00%、12.00%,癌旁正常组织HBsAg表达阳性率明显高于肝癌组织及良性肝脏疾病组织(P<0.05),肝癌组织中HBsAg表达阳性率高于良性肝脏疾病组织(P<0.05);肝癌组织中LN与HBsAg阳性表达率呈现正相关性(P<0.05),癌旁正常组织中LN与HBsAg表达呈现正相关性(P<0.05),LN在肝癌组织、癌旁组织中的阳性表达率与血清中的水平呈现正相关性(P<0.05)。结论 肝癌及癌旁正常组织中及血清LN水平表达均呈现上调,乙型肝炎病毒的感染可能是诱发正常肝细胞发展为肝癌的主要因素,LN可能参与了肝癌发展、转移过程。Abstract: OBJECTIVE To discussion the expression and correlation of LN(laminin,LN)and hepatitis b surface antigen(HBsAg)in the tissues of HBV-related hepatocellular carcinoma,so as to provide reference for clinical diagnosis and treatment of HBV-related hepatocellular carcinoma.METHODS A total of 50 cases of patients with HBV-related hepatocellular carcinoma and 50 cases of patients with benign liver diseases from Jan.2014 to Dec.2015 in the hospital were selected.Immunohistochemical method was used to detect the expression of LN and HBsAg in tissues of hepatocellular carcinoma and benign liver diseases,and normal tissue adjacent to carcinoma,and chemiluminescence was used to determine serum LN.RESULTSIn Hepatocellular carcinoma and normal tissue adjacent to carcinoma tissue,benign liver disease,LN expression positive rate were 28.00%,8.00% and 4.00%in membrane/cytoplasm,LN expression positive rate were 66.00%、54.00% and 8.00% in hepatic sinus and new blood vessels,LN expression positive rates in membrane/cytoplasm and hepatic sinus and new blood vessels with hepatocellular carcinoma were higher than those of normal tissue adjacent to carcinoma tissue and benign liver disease(P<0.05),LN expression positive rates in hepatic sinus and new blood vessels with hepatocellular carcinoma and normal tissue adjacent to carcinoma tissue and benign liver disease were higher than those of membrane/cytoplasm(P<0.05),and serum LN in hepatocellular carcinoma were higher than that of benign liver disease(P<0.05).HBsAg expression positive rates were 42.00%,70.00% and 12.00%in hepatocellular carcinoma,normal tissue adjacent to carcinoma tissue and benign liver disease,HBsAg expression positive rate of normal tissue adjacent to carcinoma tissue was higher than that of hepatocellular carcinoma and benign liver disease(P<0.05),and HBsAg expression positive rate of hepatocellular carcinoma tissue was higher than that of benign liver disease(P<0.05).LN and HBsAg positive expression rate was positive correlated with hepatocellular carcinoma tissue(P<0.05),LN and HBsAg positive expression rate was positive correlated with normal tissue adjacent to carcinoma tissue(P<0.05),and LN the positive expression rate and serum levels of hepatocellular carcinoma tissue and normal tissue adjacent to carcinoma tissue had positive correlation(P<0.05).CONCLUSION LN in the tissues and the serum level of expression are raised with hepatocellular carcinoma tissue and normal tissue adjacent to carcinoma tissue,HBV infection may be induced to normal liver cell development as the main factors of hepatocellular carcinoma,and LN may participate in the development and the transfer process of liver cancer.
