盐酸米诺环素软膏在种植体周围感染中的应用效果及对炎症反应的控制作用研究

摘要: 目的 研究盐酸米诺环素软膏在种植体周围感染中的应用效果及对炎症反应的控制作用,以为种植体周围感染患者的治疗提供指导措施。方法 选取2013年2月-2016年4月60例种植体周围感染患者按照随机数字表法分为对照组与观察组,每组各30例,对照组进行常规种植体周围感染治疗,观察组则在对照组的基础上加用盐酸米诺环素软膏进行治疗,然后将两组患者的治疗总有效率、治疗前后的牙周指数与龈沟液炎症指标进行统计与比较,同时检测所有患者的病原菌感染情况。结果 观察组30例种植体周围感染患者的治疗总有效率为100.00%,对照组30例种植体周围感染患者的治疗总有效率为86.67%,观察组明显高于对照组,治疗后2周与4周观察组牙周指数均好于对照组,龈沟液炎症指标均低于对照组,60例患者共检出病原菌78株,其中厌氧菌检出率明显高于需氧菌,P<0.05,差异均有统计学意义。结论 盐酸米诺环素软膏在种植体周围感染中的应用效果及对炎症反应的控制作用较好,因此认为盐酸米诺环素软膏在此类患者中的应用价值较高。Abstract: OBJECTIV To observe the application effect of minocycline hydrochloride ointment on infections around implant and effect on control of inflammatory reactions so as to provide guidance for treatment of patients with infection around implant. METHODS A total of 60 patients with infections around implant who were treated in the hospital from Feb 2013 to Apr 2016 were randomly divided into the control group and the observation group, with 30 cases in each group. The control group was treated with conventional therapy for the infections around implant, while the observation group was given additional minocycline hydrochloride ointment based on the treatment of the control group. The total effective rate of treatment was observed and compared between the two groups of patients, the periodontal indexes and gingival crevicular fluid inflammation indexes were statistically analyzed and compared before and after the treatment, and the prevalence of pathogenic bacteria infection in all of the patients was determined. RESULTS The total effective rate of treatment was 100.00% among the 30 patients with infections around implant in the observation group, significantly higher than 86.67% among the 30 patients with infections around implant in the control group. The periodontal indexes of the observation group were better than those of the control group after the treatment for 2 and 4 weeks; the gingival crevicular fluid inflammation indexes of the observation group were lower than those of the control group. Totally 78 strains of pathogens were isolated from the 60 patients; the isolation rate of anaerobic bacteria was significantly higher than that of the aerobic bacteria (P<0.05). CONCLUSION Minocycline hydrochloride ointment shows remarkable effect on treatment of the infection around implant and on control of inflammatory reactions, and it has high application value in treatment of this group of patients.
