白血病患者PICC相关性血流感染目标性监测及危险因素分析

Targeted monitoring and risk factors of PICC related bloodstream infections in leukemia patients

  • 摘要: 目的 探讨白血病患者外周中心静脉导管(PICC)相关性血流感染的危险因素。方法 选择2015年6月-2016年12月医院收治的白血病PICC置管患者150例,统计PICC相关性血流感染患者临床资料,经单因素及Logistic多因素分析,探讨PICC置管后发生相关性血流感染的危险因素。结果 150例患者中出现导管相关性血流感染20例,感染率13.33%,共检出病原菌20株,其中革兰阴性菌10株占50.00%,以肺炎克雷伯菌和大肠埃希菌为主;革兰阳性菌10株占50.00%,以表皮葡萄球菌为主;多因素logistic回归分析结果显示,年龄≥65岁、外周血象<2.0×109/L、穿刺次数≥6次、化疗次数≥11次、留置时间≥11个月、伴有糖尿病是PICC导管相关性血流感染的独立危险因素(P<0.05)。结论 导致白血病患者PICC相关性血流感染的原因较多,应针对危险因素,化疗前应积极处理合并症,选择合理的导管种类、穿刺和封管方式,根据药敏试验科学使用抗菌药物,缩短置管时间,从而减少PICC相关性血流感染风险。

     

    Abstract: OBJECTIVE To investigate the risk factors of PICC related bloodstream infections in leukemia patients. METHODS A total of 150 cases of leukemia patients with PICC catheter from Jun. 2015 to Dec. 2016 were selected.The clinical information of patients with catheter-related bloodstream infection were statistically analyzed, and the risk factors of PICC related bloodstream infection were analyzed by univariate analysis and multivariable logistic analysis. RESULTS There were 20 cases of PICC related bloodstream infections in 150 cases of patients, with the infection rate of 13.33%. A total of 20 strains of pathogens were found, of which 10 were gram-negative bacteria, accounting for 50%, mainly Klebsiella pneumoniae and Escherichia coli, and 10 were gram-positive bacteria, accounting for 50%, mainly Staphylococcus epidermidis. Multivariate logistic regression analysis showed that age more than 65 years old, peripheral blood less than 2×109/L, more than 6 times of puncture, more than 11 times of chemotherapy, the indwelling time of more than 11 months,and with diabetes were independent factors of PICC catheter-related bloodstream infections (P<0.05). CONCLUSION There are many reasons for PICC related bloodstream infections in leukemia patients , we should actively treat complications before chemotherapy according to the risk factors, choose reasonable catheter type and puncture and sealing methods, scientifically use antibiotics according to susceptibility test, and shorten the time of catheter, thereby reducing the risk of PICC related bloodstream infections.
