X线和多层CT及MRI对胸腰椎体结核感染的诊断效果研究

Effects of X-ray, multi-slice CT and MRI on diagnosis of thoracolumbar vertebral tuberculosis

  • 摘要: 目的 比较分析X线、多层CT、MRI对胸腰椎体结核感染的诊断效果。方法 选取2012年12月-2016年12月医院就诊的82例脊柱结核患者(208个患椎)为研究对象,均进行X线、多层CT、MRI检查,并比较三种检测方法的准确度及敏感度。结果 X线片表现为椎体破坏、椎旁脓肿、椎间隙狭窄或破坏等,CT主要表现为椎旁脓肿、椎体及附件骨质破坏、死骨等,MRI主要表现为椎间盘信号变化、椎体和附件骨破坏、椎旁脓肿、脊髓受累和韧带扩散等,多层CT和MRI对椎体破坏、椎管受累、椎旁脓肿和椎间盘受累的阳性检出率高于X线(P<0.05);多层CT对死骨形成的阳性检出率和平均评分高于X线和MRI(P<0.05);MRI对椎间盘受累、椎管受累、椎体破坏和椎旁脓肿的阳性检出率和平均评分高于多层CT(P<0.05);多层CT、MRI病灶检出时间低于X线(P<0.05),检查所需时间高于X线(P<0.05);MRI检查所需时间高于CT(P<0.05)。结论 多层CT和MRI结合互补用于脊柱结核的早期诊断,有助于提高早期诊断率,对脊柱结核的临床治疗有十分重要的意义。

     

    Abstract: OBJECTIVE To compare the effects of X-ray, multi-slice CT and MRI on diagnosis of thoracolumbar vertebral tuberculosis.METHODS A total of 82 patients with spinal tuberculosis (208 vertebrae) who were treated in the hospital from Dec 2012 to Dec 2016 were recruited as the study objects and received X-ray, multi-slice CT and MRI examinations, and the accuracy and sensitivity were observed and compared among the three examination methods.RESULTS The X-ray showed vertebral destruction, paraspinal abscess, intervertebral space stenosis or destruction. The CT mainly showed paravertebral abscess, destruction of vertebral body and accessories bone and sequestrum. The MRI mainly showed change of intervertebral disc signal, destruction of vertebral and accessories bone, paraspinal abscess, spinal cord involvement and ligament spread. The positive rates of detection of vertebral body destruction, spinal canal involvement, paraspinal abscess and intervertebral disc involvement were higher by the multi-slice CT and MRI than by the X-ray (P<0.05). The positive rate of detection of sequestration and average score were higher by the multi-slice CT than by the X-ray and MRI (P<0.05). The positive rates of detection of intervertebral disc involvement, spinal canal involvement, vertebral body destruction and paraspinal abscess and average score were higher by the MRI than by the multi-slice CT (P<0.05). The time of detection of foci was shorter by the multi-slice CT and MRI than by the X-ray, while the examination time was longer by the multi-slice CT and MRI than by the X-ray (P<0.05); the examination time was longer by the MRI than by the CT (P<0.05).CONCLUSION Multi-slice CT combined with MRI, as is used for early diagnosis of spinal tuberculosis, may raise the rate of early diagnosis and have great significance in clinical treatment of the spinal tuberculosis.
