烧伤患者细菌感染特征分析及相关因素分析

Characteristics and related factors for bacterial infections in burn patients

  • 摘要: 目的 了解医院烧伤整形科烧伤患者的医院感染(NI)率、相关因素及病原菌分布、抗菌药物敏感性。方法回顾性选取 2014年1月-2017年1月医院烧伤整形科病房烧伤患者85例,监测患者感染病原菌分布及药物敏感试验。结果 85例烧伤患者中61发生医院感染,感染率为71.8%,61例患者共检出病原菌78株,其中革兰阴性菌64株占82.0%,革兰阳性菌12株占15.4%,真菌2株占2.6%;铜绿假单胞菌对美罗培南的敏感率最高为94.1%,鲍曼不动杆菌对奈替米星敏感率最高为81.5%;多因素Logistic回归分析显示,TBSA> 40%、全层烧伤、年龄≥65岁和吸入性损伤被确定为获得NI的相关因素。结论 烧伤患者发生医院感染的相关因素较多,铜绿假单胞菌为医院感染最常见病原菌。

     

    Abstract: OBJECTIVE To investigate the incidence of nosocomial infections (NI) in burn patients of burns and plastic surgery department and related factors for the NI and observe the distribution and drug susceptibility of pathogens.METHODS A total of 85 burn patients who were treated in wards of burns and plastic surgery department from Jan 2014 to Jan 2017 were retrospectively enrolled in the study. The distribution and drug susceptibility rates of pathogens causing the infections were observed.RESULTS Of the 85 burn patients, 61 had NI, with the infection rate 71.8%. Totally 78 strains of pathogens were isolated from the 61 patients, 64 (82.0%) of which were gram-negative bacteria, 12 (15.4%) were gram-positive bacteria, and 2 (2.6%) were fungi. The drug susceptibility rate of Pseudomonas aeruginosa strains to meropenem was the highest (94.1%), and the drug susceptibility rate of Acinetobacter baumannii strains to netilmicin was the highest (81.5%). Multivariate logistic regression analysis indicated that TBSA more than 40%, full-thickness burns, no less than 65 years of age and inhalation injury were defined as the related factors for the NI.CONCLUSION There are a variety of related factors for the NI in the burn patients, and the P.aeruginosa is the most common species of pathogen causing the NI.
