Abstract:
OBJECTIVE To explore the effect of endovascular aneurysm repair (EVAR) on clinical prognosis and aortic remodeling of type B aortic dissection patients complicated with infections so as to provide guidance for clinical treatment.
METHODS A total of 117 type B aortic dissection patients complicated with infections who were treated in hospitals from Feb 2013 to Dec 2016 were enrolled in the study and divided into two groups according to treatment approach.The control group with 56 cases was given single drug therapy, while the study group with 61 cases was treated with EVAR, the follow-up was conducted after the surgery for 3 months-2 years.The incidence of complications and mortality rate were compared between the two groups of patients, and the status of aortic remodeling was analyzed by means of CT angiography.
RESULTS The incidence rates of complications such as endoleak, inflammatory syndrome, SG displacement, renal insufficiency and thrombosis of the study group were respectively 8.20%(5/61), 16.39%(10/61), 1.64%(1/61), 13.11%(8/61) and 3.28%(2/61), significantly lower than those of the control group (all
P<0.05).The mortality rate of the study group was 0 (0 case) after the treatment for 3 months, 3.28% (2 cases) after the treatment for 1 year, 6.5% (4 cases) after the treatment for 2 years, significantly lower than that of the control group (all
P<0.05).The diameters of both true and false aortic lumens have changed after the treatment for 3 months-2 years, the diameters of true aortic lumens were increased after the treatment, while the diameters of false aortic lumens were decreased after the treatment, and the study group was significantly superior to the control group in the range of variation (all
P<0.05).
CONCLUSION The EVAR is safe and effective in clinical treatment of the type B aortic dissection patients complicated with infections, facilitates the remodeling of the aortic lumens and has positive effect on the prognosis of the patients, with the incidence of complications and mortality rate low.
