乳腺手术预防性使用抗菌药物与术后切口感染相关性分析
Correlation between prophylactic use of antibiotics and postoperative incision infection in breast surgery patients

摘要: 目的 探讨乳腺手术中预防性使用抗菌药物与术后切口感染的相关性。方法 选择2012年1月-2016年12月医院进行非感染性乳腺手术患者100例,按照患者是否预防性使用抗菌药物分为对照组和观察组,每组50例,其中对照组未使用抗菌药物,观察组使用抗菌药物,对比治疗的效果。结果 两组患者的住院时间,成本-效果比,切口感染情况数据差异均不具有统计学意义,观察组患者的费用消耗高于对照组,数据差异具有统计学意义(P<0.05),两组患者在开始手术前,手术后6小时,手术后24小时,手术后48小时,手术后72小时五个时间点的白细胞计数与体温变化趋势一致。结论 乳腺手术患者预防性使用抗菌药物与手术后切口感染并没有直接关联,非感染性乳腺手术抗菌药物的使用需要符合用药原则,对于降低患者的治疗费用具有一定的意义。Abstract: OBJECTIVE To explore the correlation between the intraoperative antibiotics prophylaxis and the postoperative incision infection in the breast surgery patients. METHODS A total of 100 patients who received non-infectious breast surgery in the hospital from Jan 2012 to Dec 2016 were enrolled in the study and divided into the control group and the observation group according the status of antibiotics prophylaxis, with 50 cases in each group. The control group was not treated with antibiotics, while the observation group was treated with antibiotics. The curative effects were observed and compared between the two groups. RESULTS There were no significant differences in the length of hospital stay, cost-effectiveness ratio and incidence of incision infection between the two groups of patients. The costs of the observation group were significantly more than those of the control group (P<0.05). The white blood cells counts and body temperature of the observation group changed as same as those of the control group before the surgery and after the surgery for 6, 24 and 48 hours. CONCLUSIONS The prophylactic use of antibiotics is not directly associated with the postoperative incision infection in the breast surgery patients, the antibiotics should be used for the non-infectious breast surgery patients based on the administration indications, which has certain significance in cutting the treatment cost.
