Abstract:
OBJECTIVE To compare and analyze the differences in antimicrobial stewardship programs(ASP) at home and abroad so as to to put forward some suggestions for the implementation of ASP in China.
METHODS Based on the database retrieval of Pubmed, CNKI, and so on, bibliometric, policy analysis and retrospective literature analysis were taken as the major research methods to visualize the differences in the ASP at home and abroad.
RESULTS The foreign antibacterial drug management project was ASP-oriented and started earlier than that in China.The research content of ASP abroad covered clinical treatment, project strategy optimization and project effectiveness evaluation.There were various research methods including large sample status survey, class experiment research, cohort study, cost-benefit analysis and so on.However, domestic AMS-based ASP was at the stage of exploration and development, the research focused on reasonable use of antibiotics, type I incision and nosocomial infections.The research on project evaluation was relatively simple in China, which was mainly based on outpatient prescriptions, utilization rate of antibiotics, intensity of use and proportion of type I incision surgery for prophylactic use of antibiotics.
CONCLUSION Drawing on the foreign experience while integrating the actual situation of China, it is suggested to establish the AMS strategy of multi-disciplinary cooperation model with information technology support, change from passive acceptance of antibacterial drug regulation to early warning of antibacterial drug use, and realize the safe management of antibiotics under the support of information system.It is necessary to explore macro assessment, horizontal assessment and longitudinal assessment of the "Trinity" antibacterial drug management project evaluation mechanism and constantly optimize the management of antibiotics so as to achieve the dynamic management.
