阿维巴坦联合头孢他啶或氨曲南对耐碳青霉烯肠杆菌科细菌体外抗菌活性研究

In vitro antimicrobial activities of ceftazidime-avibactam or aztreonam-avibactam against carbapenem-resistant Enterobacteriaceae

  • 摘要: 目的 探讨阿维巴坦(Avibactam, AVI)与头孢他啶(ceftazidime,CAZ)、氨曲南(aztreonam,ATM)联用,对耐碳青霉烯肠杆菌科细菌(Carbapenem-resistant Enterbacteriaceae,CRE)的抗菌活性。方法 收集大连医科大学附属第一医院2015-2018年临床分离CRE菌株共143株,应用微量肉汤稀释法测定头孢他啶/阿维巴坦(CAZ/AVI),氨曲南/阿维巴坦(ATM/AVI)最小抑菌浓度(MIC),用PCR方法检测超广谱β-内酰胺酶(ESBLs),AmpC酶以及碳青霉烯酶相关耐药基因。结果 143株CRE菌株,CAZ/AVI MIC50为8/4μg/ml,与CAZ比较MIC50下降128倍,MIC90无变化; ATM/AVI MIC50为1/4μg/ml,MIC90为4/4μg/ml,与ATM比较,分别下降512倍和256倍。对于产KPC酶的CRE菌株,CAZ/AVI的MIC50为2/4μg/ml, MIC90为8/4μg/ml,与CAZ比较MIC50和MIC90分别下降512倍和256倍; ATM/AVI的MIC50和MIC90分别为1/4μg/ml和4/4μg/ml,与ATM比较MIC50和MIC90分别下降512倍和256倍。对于产NDM金属酶的CRE菌株,CAZ/AVI MIC50和MIC90分别为1 024/4μg/ml和2 048/4μg/ml,与CAZ比较无变化;而ATM/AVI的MIC50和MIC90分别为0.5/4μg/ml和2/4μg/ml,与ATM相比较MIC50和MIC90分别下降1 024倍及512倍。结论 头孢他啶/阿维巴坦对于产KPC酶的CRE菌株具有较强的抗菌效果,对产NDM金属酶的CRE菌株无抑制作用;氨曲南/阿维巴坦对于产KPC酶和NDM金属酶的CRE菌株均具有良好的抗菌作用。

     

    Abstract: OBJECTIVE To explore the antibacterial activity of avibactam(AVI) combined with ceftazidime(CAZ) and aztreonam(ATM) against carbapenem-resistant Enterobacteriaceae(CRE). METHODS A total of 143 strains of CRE were isolated from The First Affiliated Hospital of Da Lian Medical University from 2015 to 2018, the minimal inhibitory concentrations(MICs) of ceftazidime-avibactam(CAZ-AVI) and aztreonam-avibactam(ATM-AVI) were determined by using micro broth dilution method, and the extended-spectrum beta-lactamases-(ESBLs), AmpC-and carbapenemase-related resistance genes were detected by means of PCR. RESULTS Among the 143 strains of CRE, the MIC50 of CAZ-AVI was 8/4 μg/ml, 128 times low as that of CAZ, while the MIC90 did not change.The MIC50 of ATM-AVI was 1/4 μg/ml, 512 times low as that of ATM; the MIC90 of ATM-AVI was 4/4 μg/ml,256 times low as that of ATM.As for the KPC-producing CRE strains, the MIC50 of CAZ-AVI was 2/4 μg/ml, 512 times low as that of CAZ; the MIC90 of CAZ-AVI was 8/4 μg/ml, 256 times low as that of CAZ.The MIC50 of ATM-AVI was 1/4 μg/ml, 512 times low at that of ATM; the MIC90 of ATM-AVI was 4/4 μg/ml, 256 times low as that of ATM.As for the NDM-producing CRE strains, the MIC50 of CAZ-AVI was 1024/4 μg/ml, the MIC90 2048/4 μg/ml; the MIC50 and MIC90 of CAZ did not change as compared with CAZ-AVI.The MIC50 of ATM-AVI was 0.5/4 μg/ml, 1 024 times low as that of ATM; the MIC90 of ATM-AVI was 2/4 μg/ml, 512 times low as that of ATM. CONCLUSION CAZ-AVI can achieve remarkable antibacterial effect on the KPC-producing CRE strains but does not have inhibitory effect on the NDM-producing CRE strains.ATM-AVI has favorable antibacterial effect on both the KPC-and NDM-producing CRE strains.
