儿童肺炎支原体感染的危险因素及IL-6/STAT3通路表达

Risk factors for Mycoplasma pneumoniae infection in children and expressions of IL-6/STAT3 pathways

    目的 分析儿童肺炎支原体感染危险因素及白细胞介素-6(IL-6)/信号传导与转录激活因子3(STAT3)通路的表达。
    方法 选取2021年4月-2023年4月济南市第二妇幼保健院收治的73例肺炎支原体感染患儿为研究组, 同期健康体检儿童73名为对照组, 比较两组临床资料, 分析儿童肺炎支原体感染危险因素;受试者工作特征(ROC)曲线分析IL-6、STAT3、WBC、C-反应蛋白(CRP)、降钙素原(PCT)对儿童肺炎支原体感染的诊断价值。
    结果 有反复上呼吸道感染史和集体生活是儿童肺炎支原体感染的危险因素(OR=1.988、2.123, P<0.05);研究组IL-6、STAT3 mRNA、WBC、CRP、PCT水平高于对照组(P<0.05);ROC曲线结果显示, IL-6、STAT3、WBC、CRP、PCT水平单独及联合检测诊断儿童肺炎支原体感染的曲线下面积(AUC)分别为0.829、0.813、0.852、0.759、0.849和0.953, 联合检测的敏感度和特异度为86.30%和90.40%。
    结论 儿童肺炎支原体感染IL-6/STAT3通路被激活, 上调WBC、CRP和PCT表达;IL-6、STAT3、WBC、CRP、PCT联合检测有助于诊断儿童肺炎支原体感染, 反复上呼吸道感染史和集体生活增加儿童肺炎支原体感染的风险。

     

    Abstract:
    OBJECTIVE To analyze the risk factors for Mycoplasma pneumoniae infection in the children and observe the expressions of interleukin-6 (IL-6)/ signal transducer and activator of transcription 3 (STAT3) pathways.
    METHODS A total of 73 children with M. pneumoniae infection who were treated in the Jinan Second Maternal and Child Health Hospital from Apr 2021 to Apr 2023 were assigned as the study group, meanwhile, 73 healthy children who received physical examination were chosen as the control group. The clinical data were compared between the two groups, the risk factors for the M. pneumoniae infection were analyzed, and the values of IL-6, STAT3, WBC, C-reactive protein (CRP) and procalcitonin (PCT) in diagnosis of the M. pneumoniae infection were analyzed by means of receiver operating characteristic (ROC) curves.
    RESULTS The history of recurrent upper respiratory tract infection and living together were the risk factors for the M. pneumoniae infection in the children(OR=1.988, 2.123, P < 0.05). The levels of IL-6, STAT3 mRNA, WBC, CRP and PCT of the study group were higher than those of the control group(P < 0.05). The result of ROC curve analysis showed that the areas under curves (AUCs) of IL-6, STAT3, WBC, CRP, PCT and the joint detection of the indexes were respectively 0.829, 0.813, 0.852, 0.759, 0.849 and 0.953 in diagnosis of M. pneumoniae infection in the children; the sensitivity of the joint detection of the indexes was 86.30%, the specificity 90.40%.
    CONCLUSION The IL-6/STAT3 pathways of the children with M. pneumoniae infection are activated, the expression levels of WBC, CRP and PCT are upregulated. The joint detection of IL-6, STAT3, WBC, CRP and PCT may facilitate the diagnosis of M. pneumoniae infection in the children. The history of recurrent upper respiratory tract infection and living together may increase the risk of M. pneumoniae infection in the children.
