MCP-1基因多态性与学龄前儿童下呼吸道感染的关联及其危险因素

Association between MCP-1 gene polymorphisms and lower respiratory tract infection in preschool children and risk factors

    目的 分析学龄前儿童下呼吸道感染危险因素及与单核细胞趋化蛋白-1(MCP-1)基因多态性的关联。
    方法 选取山东第一医科大学附属人民医院2021年3月-2023年3月治疗的下呼吸道感染学龄前患儿126例为研究组, 152名同期健康体检学龄前儿童为对照组。分析下呼吸道感染病原菌分布, 多因素分析学龄前儿童下呼吸道感染的危险因素, 比较两组MCP-1基因2518位点A/G基因分布。
    结果 感染患儿共分离出128株病原菌, 革兰阴性菌(88株占68.75%), 革兰阳性菌(40株占31.25%);哮喘、早产、每周零食摄入≥7次、家长吸烟、低出生体质量、外周血MCP-1表达水平较高均为学龄前下呼吸道感染的独立危险因素(P<0.05);两组研究对象MCP-1基因2518位点A/G基因型及等位基因频率比较差异无统计学意义。
    结论 学龄前儿童下呼吸道感染病原菌以流感嗜血杆菌、金黄色葡萄球菌为主, 与哮喘、早产、每周零食摄入≥7次、家长吸烟、低出生体质量、外周血MCP-1表达水平密切相关, 与MCP-1基因分布无相关性。

     

    Abstract:
    OBJECTIVE To explore the risk factors for the lower respiratory tract infection in the preschool children and observe the association with monocyte chemoattractant protein-1 (MCP-1) gene polymorphisms.
    METHODS A total of 126 preschool children with lower respiratory tract infection who were treated in Shandong First Medical University Affiliated People′s Hospital from Mar 2021 to Mar 2023 were assigned as the study group, meanwhile, 152 healthy preschool children who received physical examination were chosen as the control group. The distribution of pathogens causing the lower respiratory tract infection was analyzed, multivariate analysis was performed for the risk factors for the lower respiratory tract infection, and the frequencies of A/G at 2518 locus of MCP-1 gene were observed and compared between the two groups.
    RESULTS Totally 128 strains of pathogens were isolated from the children with infection, 88 (68.75%) of which were gram-negative bacteria, 40 (31.25%) were gram-positive bacteria. Asthma, premature delivery, weekly intake of snack food no less than 7 time, parents′ smoking, low birth weight and high expression levels of peripheral blood MCP-1 were the independent risk factors for the lower respiratory tract infection in the preschool children(P < 0.05). There were no significant differences in the frequencies of A/G genotypes and alleles at 2518 locus of MCP-1 gene between the two groups of research subjects.
    CONCLUSION Haemophilus influenzae and Staphylococcus aureus are the predominant species of pathogens isolated from the preschool children with lower respiratory tract infection, and the infection is closely associated with the asthma, premature delivery, weekly intake of snack food no less than 7 time, parents′ smoking, low birth weight and expression levels of peripheral blood MCP-1 but is not associated with MCP-gene.
