腺病毒肺炎和肺炎支原体肺炎患儿血清及肺泡灌洗液中炎症细胞因子水平

Inflammatory cytokines in serum and alveolar lavage fluid of children with adenovirus pneumonia and Mycoplasma pneumoniae

    目的 探讨腺病毒肺炎和肺炎支原体肺炎患儿血清及肺泡灌洗液中炎症细胞因子。
    方法 收集2022年1月-2023年1月南阳医学高等专科学校第一附属医院诊治的腺病毒肺炎(腺病毒组)和肺炎支原体肺炎(肺炎支原体组)各40例患儿的临床资料,入院时均行血清及肺泡灌洗液促炎细胞因子白细胞介素-2(IL-2)、IL-6、IL-8、肿瘤坏死因子-α(TNF-α)及抗炎细胞因子IL-4、IL-5、IL-10的检测并进行分析。
    结果 两组患儿血清及肺泡灌洗液中促炎细胞因子IL-2、IL-8、TNF-α差异无统计学意义,IL-6水平腺病毒组高于肺炎支原体组(P<0.05);两组患儿血清及肺泡灌洗液中抗炎细胞因子IL-4水平差异无统计学意义,腺病毒组IL-5、IL-10高于肺炎支原体组(P<0.05)。
    结论 腺病毒肺炎与肺炎支原体肺炎患儿血清及肺炎灌洗液中炎症细胞因子表达有相同之处也有不同之处,分析不同的细胞因子表达状况,对于推测及判断感染病原体有一定的临床意义。

     

    Abstract:
    OBJECTIVE To investigate the inflammatory cytokines in serum and alveolar lavage fluid of children with adenovirus pneumonia and Mycoplasma pneumoniae.
    METHODS Clinical data of 40 children each with adenovirus pneumonia (adenovirus group) and Mycoplasma pneumoniae (Mycoplasma pneumoniae group) diagnosed and treated in the First Affiliated Hospital of Nanyang medical college form Jan. 2022 to Jan. 2023 were collected, and pro-inflammatory cytokines interleukin-2 (IL-2), IL-6, IL-8, tumor necrosis factor-α (TNF-α), and anti-inflammatory cytokines IL-4, IL-5 and IL-10 in serum and alveolar lavage fluid on admission were detected and analyzed.
    RESULTS There were no statistically significant differences in the pro-inflammatory cytokines IL-2, IL-8 and TNF-α, the level of IL-6 in the serum and alveolar lavage fluid between the two groups, and IL-6 in the adenovirus group was higher than that in the Mycoplasma pneumoniae group (P < 0.05). There was no significant difference in the level of anti-inflammatory cytokine IL-4 in serum and alveolar lavage fluid between the two groups, and the level of IL-5 and IL-10 in the adenovirus group was higher than that in Mycoplasma pneumoniae group (P < 0.05).
    CONCLUSION There were similarities and differences in the expression of inflammatory cytokines in the serum and pneumonia lavage fluid of children with adenovirus pneumonia and Mycoplasma pneumoniae pneumonia, and the analysis of the different cytokine expression status was of clinical significance for speculating and judging the differences in the pathogens of infection.
