医院传染病应急管理中“医院感染专盯”岗位的实践探索

Exploration of practice of "specialist in nosocomial control" position in emergency management of infectious diseases in hospitals

    目的 探讨“医院感染专盯”岗位在医疗机构应对传染病疫情应急中的实践及应用效果。
    方法 以首都医科大学附属北京地坛医院隔离病房医护人员为研究对象,通过对比“医院感染专盯”岗位设立前后医院感染管理重点监测指标及医院感染发生率等,结合定量、定性分析方法验证该岗位设置在感染控制管理工作中的实践效果和应用意义。
    结果 “医院感染专盯”岗位设立后,应急区医护人员感染知识、医院感染知识评分较设立前提升(P<0.05);手卫生依从率,口罩佩戴及防护服穿脱合格率较前改善(P<0.05),医护及患者感染风险也有所降低。
    结论 在新冠等呼吸道传染病应急管理中,医院结合实际情况设置并推进“医院感染专盯”岗位,对于保证应急区医疗安全质量、降低医院感染事件发生具有重要应用价值,也对未来不明呼吸道传染病的应急处置管理提供借鉴与思考。

     

    Abstract:
    OBJECTIVE To explore the practice and application effect of "specialist in nosocomial control" in medical institutions in response to infectious disease outbreaks and emergencies.
    METHODS Taking the medical and nursing staff of the isolation ward of Beijing Ditan Hospital affiliated with Capital Medical University as the research subject, the practical effect and application significance of the post setting in the management of the infection control were verified by comparing the key indicators of hospital infection management and the incidence rate of hospital infection before and after the establishment of the post of "specialist in nosocomial control", and by combining the quantitative and qualitative analysis methods.
    RESULTS After the establishment of "specialist in nosocomial control" post, the scores of infection knowledge and hospital sense knowledge of healthcare workers in the emergency zone improved significantly compared with those before the established of the post (P < 0.05). In addition, the rates of hand hygiene compliance and correctness, and the pass rates for mask wearing and donning and doffing of protective clothing improved compared with those before the established of the post (P < 0.05), and the risk of infection of healthcare workers and patients was also reduced.
    CONCLUSION In the emergency management of COVID-19 respiratory infectious diseases, the post of "hospital infection special attention" was set up and promoted according to the actual situation, which was of great value to ensure the quality of medical safety in the emergency area and reduce the occurrence of nosocomial infection, and it also provided reference and thinking for the management of the emergency response of unknown respiratory infectious diseases in the future.
