血液透析患者导管相关感染危险因素及风险预测模型研究进展

Progress of research on risk factors for catheter-associated infections in hemodialysis patients and risk prediction model

  • 摘要: 血液透析导管相关性血流感染(CRBSI)已成为血液透析患者中最常见的并发症之一。研究发现, 继心脑血管疾病之后, 感染是终末期肾病患者死亡的第二大原因。年龄≥65岁、合并糖尿病、血清白蛋白(ALB) < 35 g/L、置管时间≥7 d等因素均被认为是血液透析患者发生CRBSI的独立危险因素。基于这些发现, 建立的风险预测模型及监测手段能够早期识别血透患者中的血流感染, 这对于临床医生及时向患者提供适当的预防和治疗措施来说至关重要, 不仅有助于降低血透患者因血流感染导致的病死率, 还能减轻其疾病负担。本文旨在全面了解国内外影响血液透析患者CRBSI的危险因素、病原学及其风险预测相关内容, 并围绕该内容进行综述。

     

    Abstract: Catheter Related Blood Stream Infection (CRBSI) has become one of the most common complications in hemodialysis patients. It has been found that after cardiovascular and cerebrovascular diseases, infections are the second leading cause of death in patients with end-stage renal disease. Age ≥65 years, comorbid diabetes mellitus, serum albumin (ALB) < 35 g/L, and duration of catheter ≥7 days were identified as independent risk factors for CRBSI in hemodialysis patients. Based on these findings, risk prediction models and monitoring tools have been developed for early identification, which is crucial for clinicians to provide appropriate preventive and therapeutic measures in time, and for hemodialysis patients not only to help reduce the mortality rate due to CRBSI, but also to reduce their disease burden. The aim of this article is to provide a comprehensive understanding and review of the risk factors, etiology and their risk prediction related to CRBSI in domestic and foreign researches.
