Nrf2 mRNA、HO-1 mRNA与糖尿病感染性角膜炎患者病情严重程度及预后的关联

Association of Nrf2 mRNA and HO-1 mRNA with disease severity and prognosis in diabetic patients with infectious keratitis

    目的 分析2型糖尿病感染性角膜炎患者病原学特点, 探讨泪液中核因子E2相关因子2mRNA(Nrf2mRNA)、血红素氧合酶-1mRNA(HO-1mRNA)与病情严重程度及预后的关联。
    方法 纳入2020年1月-2024年6月新乡市中心医院收治的63例2型糖尿病感染性角膜炎患者为研究对象, 分析患者的病原菌特征;按病情严重程度分轻度组(n=32)、中度组(n=21)及重度组(n=10)。比较不同病情组Nrf2mRNA、HO-1mRNA表达, 用Spearman相关系数分析其与病情严重程度的相关性。按治疗结果分预后良好组(n=51)与预后不良组(n=12), 比较两组Nrf2mRNA、HO-1mRNA表达水平, 绘制受试者工作特征曲线(ROC)分析其对预后不良的预测价值。
    结果 共检出病原菌71株, 革兰阳性菌38株(53.52%), 以表皮葡萄球菌为主;革兰阴性菌21株(29.58%), 主要为铜绿假单胞菌;真菌12株(16.90%), 主要为曲霉菌属。不同病情组Nrf2 mRNA、HO-1 mRNA表达:重度组<中度组<轻度组(P<0.05), 且二者表达与病情严重程度呈负相关(P<0.05)。预后不良组Nrf2 mRNA、HO-1 mRNA表达均低于预后良好组(均P<0.05);ROC曲线分析显示, Nrf2 mRNA、HO-1 mRNA表达及联合检测的曲线下面积(AUC)分别为0.879、0.900和0.953, 联合检测优于单一检测(P<0.05)。
    结论 2型糖尿病感染性角膜炎病原菌以革兰阳性菌为主, Nrf2/HO-1信号通路表达与病情及预后相关。

     

    Abstract:
    OBJECTIVE To analyze the etiological characteristics of infectious keratitis in patients with type 2 diabetes and investigate the association of nuclear factor erythroid 2-related factor 2 mRNA (Nrf2 mRNA) and heme oxygenase-1 mRNA (HO-1 mRNA) in tear fluid with disease severity and prognosis.
    METHODS A total of 63 patients with type 2 diabetes and infectious keratitis admitted to Xinxiang Central Hospital from Jan. 2020 to Jun. 2024 were enrolled. The pathogenic bacteria characteristics of the patients were analyzed. Patients were divided into mild (n=32), moderate (n=21) and severe (n=10) groups based on disease severity. The expression levels of Nrf2 mRNA and HO-1 mRNA were compared among the groups, and their correlation with disease severity was analyzed by Spearman′s correlation coefficient. According to treatment outcomes, patients were classified into a good prognosis group (n=51) and a poor prognosis group (n=12). The expression levels of Nrf2 mRNA and HO-1 mRNA were compared between the two groups, and receiver operating characteristic (ROC) curves were plotted to evaluate their predictive value for poor prognosis.
    RESULTS A total of 71 pathogenic bacteria were isolated, including 38 gram-positive bacteria (53.52%), predominantly Staphylococcus epidermidis, 21 gram-negative bacteria (29.58%), mainly Pseudomonas aeruginosa and 12 fungi (16.90%), primarily Aspergillus spp. The expression levels of Nrf2 mRNA and HO-1 mRNA followed the trend: severe group < moderate group < mild group (P < 0.05), and both were negatively correlated with disease severity (P < 0.05). The poor prognosis group exhibited lower expression of Nrf2 mRNA and HO-1 mRNA than the good prognosis group (both P < 0.05). ROC curve analysis showed that the areas under the curve (AUC) for the expression of Nrf2 mRNA, HO-1 mRNA and their combined detection were 0.879, 0.900 and 0.953, respectively, with combined detection outperforming single detection (P < 0.05).
    CONCLUSIONS Gram-positive bacteria are the predominant pathogenic bacteria in type 2 diabetes-related infectious keratitis. The expression of the Nrf2/HO-1 signaling pathway is associated with disease severity and prognosis.
