舒巴坦/度洛巴坦联合14种抗菌药物对鲍曼不动杆菌临床分离株的体外抗菌活性评估

In vitro antimicrobial activity of sulbactam-durlobactam combined with 14 types of antibiotics against clinical Acinetobacter baumannii isolates

    目的 通过3种体外联合药敏试验,评估舒巴坦/度洛巴坦联合其他常见抗菌药物对鲍曼不动杆菌的抗菌活性。
    方法 收集复旦大学附属中山医院2025年2-3月临床标本分离的鲍曼不动杆菌共84株。分别使用双纸片协同试验、肉汤纸片洗脱法和联合杀菌曲线检测舒巴坦/度洛巴坦联合其他14种常见抗菌药物的体外协同有效率。
    结果 双纸片协同试验显示舒巴坦/度洛巴坦联合其他抗菌药物的有效率由高到低依次为:头孢哌酮/舒巴坦85.71%、头孢他啶76.19%、头孢吡肟76.19%、哌拉西林/他唑巴坦72.62%。联合杀菌曲线定量测定结果显示,舒巴坦/度洛巴坦联合上述4种药物分别在6~12 h后均呈现协同作用。对12株产金属β-内酰胺酶的鲍曼不动杆菌进行舒巴坦/度洛巴坦联合氨曲南肉汤纸片洗脱法试验,结果显示联合抗菌的有效率为50.00%。
    结论 舒巴坦/度洛巴坦联合头孢哌酮/舒巴坦、头孢他啶、头孢吡肟、哌拉西林/他唑巴坦都具有良好的体外抗鲍曼不动杆菌作用,微生物实验室应联合开展药敏试验,推荐使用双纸片协同试验作为常规检测方法。

     

    Abstract:
    OBJECTIVE To evaluate the antibacterial activity of sulbactam/durlobactam combined with other common antibacterial agents against Acinetobacter baumannii through three types of in vitro combined antibacterial susceptibility tests.
    METHODS A total of 84 strains of A. baumannii isolated from clinical specimens at Zhongshan Hospital Affiliated to Fudan University from Feb. to Mar. 2025 were collected. The in vitro synergistic efficacy rates of sulbactam/durlobactam combined with 14 other common antibacterial agents were detected with the double-disk synergy test, broth disk elution method and combined killing curve assay.
    RESULTS The double-disk synergy test showed that the efficacy rates of sulbactam/durlobactam combined with other antibacterial agents, in a descending order, were as follows: cefoperazone/sulbactam 85.71%, ceftazidime 76.19%, cefepime 76.19% and piperacillin/tazobactam 72.62%. Quantitative results from the combined killing curve assay indicated that sulbactam/durlobactam combined with the mentioned four drugs all exhibited synergistic effects after 6-12 hours. The broth disk elution method test of sulbactam/durlobactam combined with aztreonam on 12 strains of metallo-β-lactamase-producing A. baumannii showed a combined antibacterial efficacy rate of 50.00%.
    CONCLUSIONS Sulbactam/durlobactam combined with cefoperazone/sulbactam, ceftazidime, cefepime and piperacillin/tazobactam all demonstrates favorable in vitro antibacterial effects against A. baumannii. Microbiology laboratories should conduct combined antibacterial susceptibility tests, and the double-disk synergy test is recommended as a routine detection method.
