基于药代动力学/药效学分析重症感染患者抗菌药物疗效影响因素的研究进展

Research progress in influencing factors for therapeutic effects of antibiotics of severe infection patients based on pharmacokinetics/pharmacodynamics analysis

  • 摘要: 重症感染患者常因全身性炎症反应易引发病理生理紊乱,使其抗菌药物的药代动力学(PK)和药效学(PD)发生显著变化,严重影响抗感染治疗效果,为临床合理用药带来极大挑战。本文系统综述此类患者体内药物清除率、分布容积、药物吸收过程及组织渗透性等关键PK参数的变化特征,重点探讨抗菌药物治疗的优化策略,包括设定个体化PK/PD靶值、调整负荷剂量与给药方式以及借助治疗药物监测实现剂量精准调控等。通过综合应用上述方案,旨在提升重症感染患者抗菌药物治疗的精准性与有效性,为临床合理用药及改善患者预后提供参考。

     

    Abstract: Patients with severe infections often suffer from systemic inflammatory responses that induce pathophysiological disturbances. These disturbances significantly alter the pharmacokinetics (PK) and pharmacodynamics (PD) of antimicrobial agents, severely compromising the efficacy of anti-infective therapies and posing substantial challenges to rational clinical medication. This review systematically summarizes the variation characteristics of key PK parameters in such patients, including clearance rate, distribution volume, absorption process and tissue permeability. It focuses on the strategies optimizing the antibiotics therapy, such as setting individualized PK/PD targets, adjusting loading dose and administration methods, and achieving precise dosage control through therapeutic drug monitoring. The comprehensive use of above programs aims to improve the precision and effectiveness of the antimicrobial therapy for the patients with severe infections so as to provide guidance for reasonable clinical use of antibiotics and improvement of prognosis of the patients.
