中药活性成分干预铁死亡治疗糖尿病足溃疡的研究进展

Research progress in treatment of diabetic foot ulcer with active ingredients of traditional Chinese medicine intervening ferroptosis

  • 摘要: 糖尿病足溃疡(DFU)是糖尿病患者常见且严重的并发症,表现为创面难愈、易感染,常进展为组织坏死和骨髓炎,是糖尿病患者致残、致死的重要原因。铁死亡是一种铁依赖性程序性细胞死亡方式,以细胞膜脂质过氧化物积聚为特征,近年来在DFU的发病机制与干预研究中受到广泛关注。研究多聚焦于铁代谢、脂质代谢及System Xc-/GSH/GPX4、NAD(P)H/FSP1/CoQ10等关键铁死亡调控通路,旨在降低DFU创面细胞中脂质过氧化、活性氧及Fe2+水平,上调血管内皮生长因子表达,促进创面愈合。中药凭借多靶点、多途径的综合调节作用,展现出系统性与整体性干预优势。本文综述铁死亡的分子机制及其在DFU中的病理作用,探讨中药活性成分干预铁死亡治疗DFU的作用机制,旨在为临床治疗提供理论依据与潜在靶点。

     

    Abstract: Diabetic foot ulcer (DFU) is a common and severe complication in patients with diabetes, characterized by difficult wound healing and susceptibility to infection, often progressing to tissue necrosis and osteomyelitis. DFU is a significant cause of disability and mortality in diabetic patients. Ferroptosis is an iron-dependent form of programmed cell death, marked by the accumulation of lipid peroxides in cell membranes, and has garnered widespread attention in recent years in the pathogenesis and intervention research of DFU. Research has primarily focused on iron metabolism, lipid metabolism and key ferroptosis regulatory pathways such as System Xc-/GSH/GPX4 and NAD(P)H/FSP1/CoQ10, aiming to reduce levels of lipid peroxidation, reactive oxygen species and Fe2+ in DFU wound cells, upregulate vascular endothelial growth factor expression, and promote wound healing. Traditional Chinese medicine, with its comprehensive regulatory effects through multiple targets and pathways, demonstrates systemic and holistic intervention advantages. This paper reviews the molecular mechanisms of ferroptosis and its pathological role in DFU, explores the mechanisms of action of active ingredients of traditional Chinese medicine in intervening in ferroptosis for the treatment of DFU, aiming to provide theoretical foundations and potential targets for clinical therapy.
