2013-2021年广州市某三甲医院非鲍曼的不动杆菌分子流行病学特征

Molecular epidemiological characteristics of non-Acinetobacter baumannii in a three-A hospital in Guangzhou from 2013 to 2021

    目的  了解2013-2021年广州市某三甲医院非鲍曼不动杆菌的分子流行病学特征,包括其物种分布、耐药表型及基因型、毒力基因特征和基因组结构特征。
    方法  对2013-2021年广东省人民医院临床分离的46株非鲍曼不动杆菌进行分析,采用纸片扩散法进行药敏试验,并利用全基因组测序进行耐药基因、毒力基因预测及泛基因组分析。
    结果  从46株非鲍曼的不动杆菌中鉴定出4个菌种,其中皮特不动杆菌(28株,60.87%)和医院不动杆菌(16株,34.78%)是主要菌种;溶血不动杆菌和赛氏不动杆菌均检出1株(各占2.17%)。非鲍曼的不动杆菌对头孢吡肟的耐药性最为突出,耐药率达21.74%,且存在较高的中介率。基因组学分析揭示了丰富的耐药基因(如blaOXA-417blaNDM-1)和毒力基因(主要涉及铁摄取和生物膜形成)。泛基因组分析显示核心基因比例低(204个),表明四种菌种之间基因组差距较大。
    结论  本研究揭示了该院非鲍曼的不动杆菌具有显著的遗传多样性和耐药基因储备,强调需借助分子技术加强其监测,以指导临床感染防控。

     

    Abstract:
    OBJECTIVE  To investigate the molecular epidemiological characteristics of non-Acinetobacter baumannii in a three-A hospital in Guangzhou from 2013 to 2021, including species distribution, drug resistance phenotype and genotype, characteristics of virulence gene and characteristics of genome structure.
    METHODS  A total of 46 clinical isolates of non-A. baumannii strains at Guangdong General Hospital from 2013 to 2021 were analyzed. Drug susceptibility testing was performed with the disk diffusion method, and whole-genome sequencing was employed to predict resistance genes and virulence genes, and conduct pan-genome analysis.
    RESULTS  Among the 46 non-A. baumannii strains, four species were identified, with Acinetobacter pittii (28 strains, 60.87%) and Acinetobacter nosocomialis (16 strains, 34.78%) being the predominant species. One strain each of Acinetobacter haemolyticus and Acinetobacter seifertii was detected (2.17% each). Non-A. baumannii strains exhibited the most prominent drug resistance to cefepime, with a resistance rate of 21.74%, alongside a high intermediate resistance rate. Genomic analysis revealed abundant resistance genes (e.g., blaOXA-417, blaNDM-1) and virulence genes (primarily involved in iron uptake and biofilm formation). Pan-genome analysis showed a low proportion of core genes (204), indicating significant genomic divergence among the four species.
    CONCLUSIONS  This study highlights the notable genetic diversity and reservoir of resistance genes in non-A. baumannii strains from this hospital, emphasizing the need for strengthening surveillance through molecular techniques to guide clinical infection prevention and control.
