以牙髓卟啉单胞菌为主的颅内多种厌氧菌感染1例并文献复习
Intracranial infections with multiple anaerobic bacteria predominated by Porphyromonas endodontalis: a case report and literature review

摘要: HT5"SS由于厌氧菌自身生长代谢的特点,脑脊液(CSF)中厌氧菌难以分离培养,通常认为厌氧菌引起的颅内感染(脑膜炎、脑脓肿等)在临床上较少见,且预后通常较差。近年来,由于检测技术水平的提高,厌氧菌颅内感染报道也逐渐增多,但同时合并多种厌氧菌的颅内感染患者在临床仍罕有报道。医院收治了一例细菌性脑膜炎患者经宏基因二代测序(mNGS)和脑脊液培养证实为牙髓卟啉单胞菌为主的颅内多种厌氧菌感染患者,在抗感染综合对症治疗后病情好转出院,未遗留任何后遗症。本文回顾分析该病例诊治过程,并复习相关文献资料,希望能为临床厌氧菌颅内感染的诊治提供借鉴。Abstract: Anaerobic bacteria are difficult to be isolated and cultured from cerebrospinal fluid (CSF) due to the growth and metabolisms characteristics of themselves. It is generally accepted that the intracranial infections caused by the anaerobic bacteria-such as meningitis and brain abscesses-are relatively rare in clinical practice and typically carry a poor prognosis. In recent years, advances in detection technologies have enabled an increasing number of reported cases of anaerobic intracranial infections; however, patients who were complicated with intracranial infections of multiple anaerobic bacteria remained exceedingly rare in clinical practice. A patient with bacterial meningitis who was confirmed with intracranial infections of multipole anaerobic bacteria predominated by P. endodontalis through metagenomic next generation sequencing (mNGS) was reviewed in the article, the patient was improved with the illness condition and discharged from the hospital without any sequela after comprehensive symptomatic treatment. The process of diagnosis and treatment of the patient was retrospectively analyzed in the article, and the related literatures were reviewed so as to provide bases for diagnosis and treatment of the clinical intracranial infections with anaerobic bacteria.
