HPV感染宫颈癌根治术患者组织LAMP3、ATF4、FOXP3表达与病理特征的关系

Relationship between expression of LAMP3, ATF4 and FOXP3 in tissues and pathological characteristics in cervical cancer patients with HPV infection undergoing radical surgery

    目的 分析人乳头瘤病毒(HPV)感染宫颈癌根治术患者组织溶酶体相关膜蛋白3(LAMP3)、转录激活因子4(ATF4)、叉头框转录因子(FOX)P3表达与病理特征的关系。
    方法 选取2019年1月-2023年1月江苏大学附属武进医院接收的116例HPV感染宫颈癌患者为研究对象,所有患者均行宫颈癌根治术,取患者肿瘤组织和癌旁组织,分别检测其中LAMP3、ATF4、FOXP3的表达情况。比较肿瘤组织与癌旁组织中LAMP3、ATF4、FOXP3阳性的表达情况,并分析肿瘤组织中蛋白表达和临床病理特征的关系。对所有患者进行为期2年的随访,根据患者是否肿瘤复发、转移、出现新病灶或死亡,将患者分为预后良好组(n=82)和预后不良组(n=34),比较两组患者LAMP3、ATF4、FOXP3的表达水平,通过logistic多因素分析评估蛋白表达与预后的关系。
    结果 肿瘤组织中LAMP3、ATF4、FOXP3的阳性表达率分别为71.55%、76.72%、74.14%,均高于癌旁组织,且FIGO分期增加、存在淋巴结转移和浸润深度更深的肿瘤中的阳性表达率更高(P<0.05)。随访结果显示,预后不良患者占29.31%(34/116),预后不良组的LAMP3、ATF4、FOXP3的阳性表达率分别为88.24%、91.18%、91.18%,均高于预后良好组(P<0.05)。logistic分析显示LAMP3、ATF4、FOXP3表达均是影响患者预后的重要指标,高表达患者预后不良的风险更高(P<0.05)。
    结论 HPV感染宫颈癌根治术患者组织LAMP3、ATF4、FOXP3表达均会升高,可能影响患者预后情况。

     

    Abstract:
    OBJECTIVE  To analyze the relationship between expression of lysosomal-associated membrane protein 3 (LAMP3), activating transcription factor 4 (ATF4) and forkhead box P3 (FOXP3) in tissues and pathological characteristics in cervical cancer patients with human papillomavirus (HPV) infection undergoing radical surgery.
    METHODS  A total of 116 patients with cervical cancer and HPV infection admitted to Changzhou Wujin Hospital from Jan. 2019 to Jan. 2023 were enrolled. All patients underwent radical surgery for cervical cancer, and tumor tissue and adjacent non-tumor tissue were collected for the detection of LAMP3, ATF4 and FOXP3 expression. The positive expression of LAMP3, ATF4 and FOXP3 in tumor tissue and adjacent non-tumor tissue was compared, and the relationship between protein expression in tumor tissue and clinicopathological characteristics was analyzed. All patients were followed up for 2 years and were divided into a good prognosis group (n=82) and a poor prognosis group (n=34) based on the occurrence of tumor recurrence, metastasis, new lesions or death. The expression levels of LAMP3, ATF4 and FOXP3 were compared between the two groups, and the relationship between protein expression and prognosis was assessed by multivariate logistic regression analysis.
    RESULTS  The positive expression rates of LAMP3, ATF4 and FOXP3 in tumor tissues were 71.55%, 76.72% and 74.14%, respectively, all of which were higher than those in adjacent non-tumor tissues. Furthermore, higher positive expression rates were observed in tumors with increased FIGO staging, lymph node metastasis and greater invasion depth (P<0.05). Follow-up results showed that 29.31% (34/116) of patients had a poor prognosis. Specifically, the positive expression rates of LAMP3, ATF4 and FOXP3 in this group were 88.24%, 91.18% and 91.18%, respectively, all significantly higher than those in the good prognosis group (P<0.05). Logistic analysis indicated that the expression of LAMP3, ATF4 and FOXP3 were important indicators affecting patient prognosis, and patients with high expression had a higher risk of poor prognosis (P<0.05).
    CONCLUSIONS  The expression of LAMP3, ATF4 and FOXP3 is upregulated in tissues from cervical cancer patients with HPV infection undergoing radical hysterectomy, which may affect patient prognosis.
