2020-2024年某院泌尿生殖道支原体感染的流行病学特征及药物敏感性

Epidemiological characteristics and drug susceptibility of urogenital Mycoplasma infections in a hospital from 2020 to 2024

  • 摘要: 目的 探讨医院泌尿生殖道支原体感染的流行病学及抗菌药物敏感性特征。方法 回顾性分析2020年1月-2024年12月广东省人民医院就诊的22 716例患者泌尿生殖道支原体培养及药敏数据,采用SPSS 22.0统计软件对数据进行分析。结果 22 716例患者中有6 723例支原体检测阳性,总阳性率为29.60%,2020-2024年患者支原体阳性率呈下降趋势(χ2=41.869,P<0.001)。女性患者的感染比例(6 055例,90.06%)高于男性(668例,9.94%)(χ2=557.422,P<0.001); >30~40岁为感染高发年龄段。单一解脲脲原体(Uu)感染是最主要的感染类型,其次为混合感染,单一人型支原体(Mh)感染比例最低。药敏试验结果显示,Uu+Mh混合感染组对10种抗菌药物的耐药性均高于Uu组(P<0.001),对环丙沙星、氧氟沙星、四环素、司帕沙星、强力霉素和交沙霉素的耐药性高于Mh组(P<0.05); Mh组对氧氟沙星、司帕沙星、红霉素、阿奇霉素、克拉霉素和罗红霉素的耐药性高于Uu组(P<0.001)。结论 女性支原体感染比例高于男性,>30~40岁为感染高发年龄段。Uu对多种抗菌药物表现出较高的敏感性,而Mh对多数药物的敏感性较低,二者混合感染的耐药性普遍高于单一感染。

     

    Abstract: OBJECTIVE To explore the epidemiological characteristics of urogenital tract Mycoplasma infections and observe the features of drug susceptibility. METHODS The data regarding the culture of urogenital tract Mycoplasma and drug susceptibility testing were collected from 22 716 patients who were treated in Guangdong Province People's Hospital from Jan. 2020 to Dec. 2024 and were retrospectively analyzed. The data were statistically analyzed by SPSS 22.0 software. RESULTS Among the 22 716 patients, 6 723 were tested positive for Mycoplasma, with the total positive rate 29.60%; the positive rate of Mycoplasma showed a downward trend in 2020-204(χ2=41.869,P<0.001). The proportion of the female patients with infections (6 055 cases,90.06%) was higher that of the male patients (668 cases,9.94%)(χ2=557.422,P<0.001); the infections were highly prevalent among the patients aged between 30 and 40 years old. The single Ureaplasma urealyticum (Uu) infection was the predominant type of infection, followed by the mixed infections; the proportion of the patients with single Mycoplasma hominis (Mh) infection was lowest. The result of drug susceptibility testing showed that the drug resistance rates to 10 types of antibiotics were higher in the Uu plus Mh mixed infections group than in the Uu group (P<0.001); the drug resistance rates to ciprofloxacin, levofloxacin, tetracycline, sparfloxacin, doxycycline and josamycin were higher in the Uu plus Mh mixed infections group than in the Mh group(P<0.05); the drug resistance rates of levofloxacin, sparfloxacin, erythromycin, azithromycin, clindamycin and roxithromycin were higher in the Mh group than in the Uu group(P<0.001). CONCLUSIONS The proportion of the female with Mycoplasma infections is higher than that of the male; the infections are highly prevalent among the patients aged between 30 and 40 years old. Uu is highly sensitive to a variety of antibiotics, the Mh is less sensitive to most of the antibiotics. The drug resistance rate of the mixed Uu and Mh infections is generally higher than that of the single infection.
