系统性红斑狼疮并发真菌感染病原菌及其危险因素

摘要: 目的 探讨系统性红斑狼疮(SLE)患者并发真菌感染的病原学特征,并分析危险因素,为临床防控工作提供参考。方法 回顾性选取2021年2月-2025年2月于空军军医大学第一附属医院临床免疫科就诊的59例SLE并发真菌感染患者为感染组,同时纳入同期146例单纯SLE患者为未感染组。统计分析感染患者的发病部位与病原菌分布,收集临床资料,运用logistic回归模型筛选SLE患者并发真菌感染的危险因素。结果 病原学分析显示,感染组患者真菌感染以口腔部位最为多见(占比33.90%),病原菌则以念珠菌属占比最高(占比37.31%)。多因素logistic回归分析结果提示,住院时间(OR=5.368)、SDI指数(OR=75.036)、SLEDAI评分(OR=2.272)以及长期用药史(OR=7.142)均为SLE患者继发真菌感染的危险因素(P<0.05)。结论 住院时间、SDI指数、SLEDAI评分和长期使用药物均是SLE并发真菌感染的危险因素,故临床治疗需密切关注相关因素评估,予以针对性管理策略,以减少SLE并发真菌感染的发生。Abstract: OBJECTIVE To explore the etiological characteristics of fungal infections in the patients with systemic lupus erythematosus (SLE) and analyze the risk factors so as to provide guidance for clinical prevention and control. METHODS A total of 59 SLE patients who were complicated with fungal infections and were treated in immunology department of the First Affiliated Hospital of the Military Medical University of the Chinese People's Liberation Army Air Force from Feb. 2021 to Feb. 2025 were retrospectively chosen as the infection group. Meanwhile, 146 patients with simple SLE were chosen as the no infection group. The infection sites and species of pathogens form the patients with infections were statistically analyzed. The clinical data were collected. Logistic regression model was employed to screen the risk factors for the fungal infections in the SLE patients. RESULTS The etiological analysis showed that the patients with oral fungal infections were dominant, accounting for 33.90%; Candida spp was the predominant pathogen, accounting for 37.31%. The result of multivariate logistic regression analysis indicated that the length of hospital stay(OR=5.368), SDI index(OR=75.036), SLEDAI score(OR=2.272) and history of long-term drug administration (OR=7.142) were the risk factors for the secondary fungal infection in the SLE patients (P<0.05). CONCLUSIONS The length of hospital stay, SDI index, SLEDAI score and long-term use of drugs are the risk factors for the fungal infections in the SLE patients. It is necessary for the hospital to carry out the assessment of relevant factors and take targeted management strategies so as to reduce the incidence of fungal infections in the SLE patients.
