极低体质量儿晚发败血症的危险因素

Risk factors for late-onset sepsis in very low birth weight infants

    目的 探讨极低出生体质量儿(VLBWI)发生晚发败血症(LOS)的危险因素。
    方法 选择2019年1月-2024年12月聊城市人民医院收治的VLBWI的临床资料进行分析,108例发生LOS的纳入LOS组,413例未发生败血症的纳入非LOS组,采用多因素logistic回归分析筛选VLBWI发生LOS的危险因素。
    结果 共纳入521例VLBWI作为研究对象,其中LOS组108例(20.73%)。革兰阴性菌为主要感染病原(61.54%),以肺炎克雷伯菌最为常见(41.03%)。LOS组28 d累积生存率较非LOS组降低(χ2=11.292,P<0.001)。多因素logistic回归分析显示,胎龄(OR=0.539,95%CI:0.415~0.700,P<0.001)、出生体质量(OR=0.019,95%CI:0.003~0.132,P<0.001)、气管插管(OR=2.849,95%CI:1.214~6.684,P=0.016)为VLBWI发生LOS的危险因素。
    结论 胎龄、出生体质量、气管插管是VLBWI发生LOS的危险因素。胎龄、出生体质量与气管插管三者联合预测模型具有较好的预测价值。临床应重视围产期管理,减少不必要的侵入性操作,以降低LOS发生风险。

     

    Abstract:
    OBJECTIVE To explore the risk factors for late-onset sepsis (LOS) in very low birth weight infants (VLBWIs).
    METHODS Clinical data of VLBWI admitted to Liaocheng People's Hospital from Jan. 2019 to Dec. 2024 were analyzed. A total of 521 VLBWIs were divided into an LOS group (n=108) and a non-LOS group (n=413) based on the presence or absence of LOS. Multivariate logistic regression analysis was conducted to screen for risk factors for LOS in VLBWI.
    RESULTS Among the 521 VLBWIs, LOS occurred in 108 cases (20.73%). Gram-negative bacteria were the primary infectious pathogens (61.54%), with Klebsiella pneumoniae being the most common (41.03%). The 28-day cumulative survival rate in the LOS group was lower than that in the non-LOS group (χ2=11.290, P<0.001). Multivariate logistic regression analysis revealed that gestational age (OR=0.539, 95%CI: 0.415-0.700, P<0.001), birth weight (OR=0.019, 95%CI: 0.003-0.132, P<0.001) and endotracheal intubation (OR=2.849, 95%CI: 1.214-6.684, P=0.016) were risk factors for LOS with VLBWIs.
    CONCLUSIONS  Gestational age, birth weight and endotracheal intubation are risk factors for LOS in VLBWIs. The combined predictive model of the three risk factors has good predictive value. Clinical attention should be paid to perinatal management to reduce unnecessary invasive procedures, thereby lowering the risk of LOS.
