Abstract:
OBJECTIVE To investigate the distribution and drug resistance of pathogens causing infection in the patients with fungal keratitis and observe the correlation between the prognosis of the patients and the genera of infectious fungi.
METHODS A total of 228 patients with fungal keratitis (228 eyes) who were treated in the third affiliated hospital of army medical university from Mar 2016 to Dec 2017 were enrolled in the study and divided into four groups according to the pathogens causing the infection: the fusarium group with 110 cases, the
Aspergillus group with 53 cases, the alternaria group with 36 cases and the other genera group with 29 cases.The distribution and drug resistance rates of the pathogens were recorded, the levels of prognosis were compared among the four groups of patients, and the medication duration and best corrected visual acuity (BCVA) were observed and compared among the cured patients.
RESULTSJP+3 Totally 242 strains of fungi were isolated from the 228 patients with fungal keratitis.The drug resistance rates of
Fusarium, Aspergillus and
Alternaria to natamycin and amphotericin B were low, while the drug resistance rates to itraconazole and fluconazole were high.There was significant difference in the prognosis level among the four groups of patients (
P<0.05), the prognosis of the patients in the fusarium group and the
Aspergillus group was poor (
P<0.05).There were significant differences in the medication duration and BCVA among the cured patients (
P<0.05).The medication duration of the cured patients was significantly longer in the fusarium group than in the
Aspergillus group, the alternaria group and the other genera group (
P<0.05).The BCVA of the cured patients was significantly higher in the alternaria group than in the
Fusarium group, the
Aspergillus group and the other genera group (
P<0.05).
CONCLUSION The
Fusarium, Aspergillus and
Alternaria are the common genera isolated from the patients with fungal keratitis.It is necessary for the hospital to reasonably use antibiotics based on the result of drug susceptibility testing, the prognosis of the patients with
Fusarium and
Aspergillus infection is poor, the medication duration of the patients with
Fusarium infection is longest, and the cured patients with
Alternaria infection have the best vision recovery.
